The filter matters more than anything you add
Coffee beans contain two oily diterpenes, cafestol and kahweol, which directly raise LDL cholesterol. How much ends up in your cup depends almost entirely on how you brew.
The measured differences are not subtle. Paper-filtered drip coffee runs a median of about 11.5 mg/L of cafestol. French press and percolator sit near 90 mg/L. Workplace brewing machines came in at a median 176 mg/L. Scandinavian-style boiled coffee measured close to 940 mg/L — roughly eighty times the paper-filtered figure. Espresso varies enormously; typical samples fall between 36 and 54 mg/L, but individual samples have tested far higher.
A paper filter removes well over 95% of it. There's a neat detail in the research here: most of the cafestol is actually retained by the coffee grounds rather than caught by the paper itself, which is why the filter's presence changes the extraction rather than just straining the result.
The effect size is real. Researchers estimated that replacing three cups a day of machine coffee with paper-filtered coffee, five days a week, would reduce LDL by 0.58 mmol/L — and lower relative cardiovascular risk by up to 13% over five years. Unfiltered coffee has been shown to raise LDL by around 8% within four weeks.
